The emergency department at Imam Al-Hujjah Hospital received an unusual case of a woman in her 30s suffering from hypovolemic shock and severe uterine bleeding. Obstetrician and gynecologist Dr. Susan Abd Al-Ameer examined the patient and discovered that she was pregnant with twins, one inside the womb and the other fetus outside the womb, which called for surgical intervention.
The patient was immediately admitted to the surgical theater under the supervision of Dr. Susan Abd Al-Ameer, anesthesiologist Dr. Abd Al-Ameer Noori, anesthetist Ahmed Wasfi, and the rest of the anesthesia assistants.
When Dr. Susan performed exploratory laparotomy she discovered severe bleeding in the patient’s abdomen. The surgical team performed a salpingostomy and treated the hemorrhage, saving the life of the fetus inside the womb. The anesthetists had a crucial role in controlling the shock and saving the patient’s life.

In the past, the patient had several miscarriages and now she is pregnant with twins after in-vitro fertilization. The patient’s condition stabilized and she was discharged from the hospital the next day after making a full recovery.
